When do you request a copy of your medical data?

Step 1

Request a copy of your medical file at Central Medical Records of Adrz. This can be accomplished with the following request forms:

• Request form to access or obtain or a copy of your medical file
• Request form to access or obtain a copy of the medical file of a child (up to age 16)
• Request/authorisation form for copy of medical file

The request form explains what exactly you can request. It is important that you complete the form in its entirety and sign it.

Step 2

Send the form by post to Adrz, Attn: Central Medical Records, PO box 15, 4460 AA Goes or by e-mail to adrz.cmd@adrz.nl.

Retrieving your data takes time. Please bear in mind that your data request will take 3-4 weeks to process.
